Introduction: Platinum-based regimens remain the standard treatment (tx) for advanced ovarian cancer (OC) in Brazil with targeted/immune tx being introduced more recently. Objective: The OCEANIA real-world study evaluated antineoplastic tx use in Brazilian patients with OC over the last 5 years. The time from diagnosis to initial tx, and the types of first-line (1L) and subsequent therapies were assessed. Methods: The retrospective cohort study assessed tx data from patients with OC who received antineoplastic tx between 2015-2020, using a Brazilian private claims database (ORIZON). Follow-up period was the time between index date and loss of follow-up/data cut-off (30June2020). Inclusion criteria include;a combination of >2 OC ICD-10 codes (C56, C57.0, C48.1, C48.2) and/or any OC-related procedure/tx. The index date (proxy for diagnosis) was defined as the first date of an OC ICD-10 code and/or an OC-related procedure/tx. 1L antineoplastic tx (first 30-day cycle) versus subsequent tx received throughout the study period (minimum 6 months) were recorded, as well as time from index date to receipt of first platinum-based drug. Results: Of 2529 OC patients identified, the majority of claims were from the Southeast (65.2%) and Northeast (18.8%). At index, mean age was 52.1 (SD=15.7) years and median follow-up was 15.7 months. Mean (SD [median]) time from index date to first platinum-based tx was 3.9 (4.6 [.2.9]) months. Of the 1441 (57.0%) patients that received antineoplastic tx, chemotherapy (CT) was the most common 1L tx (non-platinum: 1254/1441 [87.0%]; platinum-based: 1134/1441 [78.7%]). Fewer patients received targeted (20.7%), hormone (4.4%), or immune (0.4%) tx in the first 30 days. These proportions increased with subsequent tx: non-platinum-based CT (93.2%), platinum-based CT (87.0%), targeted (40.9%), hormone (4.7%), and immune (0.9%) tx. Conclusion: Over half of OC claims in this Brazilian database received anti-neoplastic tx within 3.9 months of diagnosis; CT was the most commonly used tx. Exposure to all drugs increased throughout the study period, potentially due to increased accessibility, disease recurrence and/or platinum resistance. A combined therapeutic approach using targeted/hormone/immune tx and platinum-based CT in earlier disease stages may help to minimise disease recurrence.
