The Plan of Expansion of Radiotherapy in SUS envisaged in its conception the installation of 80 linear accelerators in several cities of Brazil, aiming to fill the existing care gaps. Objective: Analysis of the current scenario of the Radiotherapy Expansion Plan in SUS proposed in 2012 comparing the planned project with the one carried out so far. Method: This was an observational cross-sectional study that included data collection and tabulation regarding the Radiotherapy Expansion Plan in the SUS through the analysis of the information available on the Ministry of Health Portal. Results: Data from project start-up in November 2015 show that the 80 solutions were expected to operate by 03/01/2018, but information from June 2019 shows that only 20 installations (25%) were completed and of these only 6 (7.5%) were inaugurated, another 6 (7.5%) were not inaugurated and 8 (10%) do not yet have an operating license, the average delay between the expected and the realized period was 730.6 days representing an average delay of 2 years. There are an additional 18 (22.5%) works in progress which have an average delay between the expected and actual deadlines of 741.5 days representing an average delay to date of 2 years with an average percentage of completion of the works in progress. 47.3% to date. Of the remaining solutions, 8 (10%) had their contracts suspended, 1 (1.25%) work paralyzed, 1 (1.25%) project under bid, 3 (3.75%) projects under preparation of the reference term, 3 (3.75%) under preparation of the executive project, 9 (11.25%) basic projects under review or review and 18 (22.5%) projects replaced by the management committee. Conclusion: The SUS Radiotherapy Expansion Plan foresaw the installation of 80 linear accelerators by March 2018, but through the analysis of the information available in June 2019 only 38 works were being executed and of these only 20 were completed with only 6 inaugurated solutions.
