IMPLEMENTATION OF PORTABLE INFUSER FOR HOME USE FOR PATIENTS WITH ADVANCED COLORECTAL CANCER WITH INDICATION OF PROTOCOLS CONTAINING 5-FLUOROURACIL IN CONTINUOUS INFUSION, AS AN ALTERNATIVE FOR COST REDUCTION IN THE UNIFIED HEALTH SYSTEM (SUS)

Introduction: Due to the difficulty in providing hospitalization beds in Brazil, in the Unified Health System (SUS), physicians often opt for less effective therapeutic regimens or with a higher toxicity profile for the treatment of Advanced Colorectal Cancer (RACC). Not provide effective and effective treatment can increase health costs. Objective: To evaluate the implantation of the Home-use Portable Infusion for patients with RACC, with indication of protocols containing 5-Fluorouracil in Continuous Infusion, and its impact on cost reduction in the SUS. Methodology: An Economic Evaluation study was conducted, though qualitative and quantitative analysis of comparison of direct costs associated with the two alternatives: Outpatient treatment with the use of portable infuser and treatment na the with the patient’s hospitalization, with the purpose of estimating the costs with the incorporation of the device into the SUS. The amounts reimbursed by the SUS for Chemotherapy for advanced colon adenocarcinoma, fees, procedures, imaging and hospitalization were obtained from the Authorization for High Cost/Complexity Procedures (APAC), Hospital Admission Authorization (AIH) and SUS Table of Procedures, Medication and OPME Management Systems (SIGTAP). Results: The costs of materials and medications for the FOLFOX6 therapeutic regimen, administered in an outpatient unit with the use of the device, had an additional cost of R$ 235.24 per treatment cycle when compared to the hospitalized patient. However, when the values reimbursed by the SUS were added with the procedures, the total cost of treatment was R$ 8,794.44 for outpatient treatment and R$ 17,796.25 for hospital treatment, equivalent to a 49.42% drop in outpatient treatment costs when compared to treatment with hospitalized patients. The total financial transfer of the complete treatment (12 cycles) for CCRA chemotherapy is R$ 13,334.00. Conclusion: The present study demonstrated that the incorporation of portable infuser for home use in therapeutic regimens containing 5-FU in continuous infusion for patients diagnosed with Advanced RCC can be a strategy to minimize the costs of cancer treatment in the SUS. Impacting on the effectiveness and effectiveness of treatment and increased quality of life of the patient.
